I am aware that a handbook and school calendar are available at the SLW website under the RSG tab. I can access them at any time. A printed copy of the the documents will be distributed during open house or on the first day of school.

I am aware that once this form is submitted, I am responsible for completing a Policy Acceptance Form and a 2025 Parental Consent & Medical Release Form. Both forms will be emailed with this registration confirmation to be completed electronically. Please take the time to review the program policies by viewing the links at slwchurch.org.

Yes, I will complete the forms promptly after they arrive. No, I will not complete the forms, therefore withdrawing my child's registration from the RSG Preschool program.

I understand that there is a $30 nonrefundable registration fee that must be paid per student registering.

My child's registration fee will be mailed to SLW - RSG P.O. Box 275 Sandy Lake, PA 16145 upon submission of registration. My child's registration fee will be paid using the Brightwheel app as soon as an invitation to link to my child's account is sent and an invoice is generated. My child's registration fee (cash or check) will be dropped off at the SLW church office in an enveloped marked with my child's full name.
